Policies

For your convenience, we have answered a variety of commonly-asked financial and policy questions below. If you need further information about any of these policies, please ask to speak with a Front Office Specialist or Practice Manager.

How may I pay?

We accept payment by cash, check, debit card, VISA, or MasterCard. For your convenience, our billing office is staffed Monday through Friday from 8:00 AM to 4:30 PM. The phone number is 770-645-0017

You are expected to make payment in full upon receipt of a billing showing your balance due. Other payment plans or options may be available upon completion of a financial statement analysis.

Please contact Billing Office for this information and/or when your billing address changes. A monthly billing charge will be added to all accounts not paid in full within 45 days of service.

When is my account delinquent?

An account is considered past due 30 days following billing unless other arrangements have been made. Unpaid accounts beyond 90 days are considered delinquent and may be forwarded to our collection agency and will have a service fee/billing fee added.

Current medication?

Please bring a list of Medication that you are taking to each visit.

How are my medication refills handled?

Our policy is for the patient to call their pharmacy and ask them to fax the request for your medication to 770-645-0224. Requests are usually handled within 48 business hours. Processing times may vary depending on the availability of your doctor, who for your safety must review each request prior to completion.

How are insurance referrals or authorizations handled?

Our policy is we will give you a referral the same day at the end of your visit if you were seen by the Doctor or Nurse Practitioner.

If you need a referral to see a specialist and the Doctor or Nurse Practitioner has not seen you for this problem before, you will need to make an appointment to see the Doctor or Nurse Practitioner before a referral will be issued.

If you have been seen or have spoken to the Doctor or Nurse Practioner about this problem and need to call back after you have set up your appointment with the specialist, please follow the instructions below:

  1. Please call the office and leave a message on the referral line.
  2. Please allow 48 hours notice for Referrals or Prior Authorizations to be completed.

Are there service charges?

If the decision is made to see a patient who does not have his/her co-pay a service charge of $30 will be added.

Excessive calls for prescriptions will be given a service charge.

There will be a billing fee of $20.00 added to each statement, billed after 75 days of service.

Is interest charged?

Patients with an outstanding balance over 60 days with be charged interest of 12%.

Phone calls or emails to Providers

Our physician and nurse practitioner will not be doing telephone or email medicine. If you need to talk your doctor we will give you an appointment.

NON-covered services

In the event that a NON-Covered service is performed: we will have no choice but to bill you directly for all charges related to your office visit. Please review your plan and know your benefits prior to your visit specifically any visits related to preventative care.

HMO Plans that require you to select a PCP

If you have an HMO plan with which we are contracted: Please make sure that your card reads Dr. Goga Vukotic or Dr. Naima Cheema in the area where the PCP name goes or make sure you have alerted your insurance company of the change prior to this appointment.

If you have not made this change with your insurance there is a chance that they will deny your charges for today’s visit and you will be responsible.

What is my financial responsibility for services?

Your financial responsibility depends on a variety of factors, claims that have not been paid in 45 days will be automatically billed to you and we can assist you in refilling your insurance at your request.

What about missed appointments?

We would appreciate your help and courtesy of a call if you are unable to keep an appointment. Please notify our office at least twenty-four(24) hours prior to the appointment time.

We reserve the right to charge you a missed appointment fee of $30 and three (3) non cancelled missed appointments are grounds for patient discharge.

Medicare

We are now Participating with Medicare and are accepting assignment.

Legal fee

Any patient sent to collections are will be responsible for all collection fees. If a patient is taken to small claims court the patient will be responsible for all fees/charges.

Emergencies after hours

If you need medical care when the office is closed, please go to the closest Emergency Room.
